Appointee Exemption Disclosure Form
Most individuals applying for appointment or reappointment to a board or commission must complete and submit an Appointee Exemption Disclosure Form. If the board you’re being appointed to requires the disclosure, the Appointments Office will contact you to submit a form. You will not move forward in the appointments process without this. Once you do submit your form, the Ethics Commission will review it and determine whether you must provide additional information, such as a financial disclosure.
